Club Fitting Questionnaire

Q&A for Client to Complete Prior to Club Fitting Session. It is necessary to ask you a number of questions about your game so that I can prepare for your visit. Your answers will give me a general idea of how you play the game and the general characteristics of what heads, and shafts in what lengths and flexes we need to test while you are here. If you are wishing to complete an in-person fitting please arrange to pick-up a copy of Tom Wishon’s “The Search for the Perfect Golf Club” and I ask that you read as much as you possibly can before the fitting session. Not only does the book give you a list of questions to ask me to insure you get the very best fitting possible, the more you read the more you will understand the process. The more you understand ahead of time the easier it will be to get the best data. Please complete the following questions to the best of your ability by either filling in the blank or selecting the correct answer. Note: This form is very important and I need to have it to review before I can confirm a fitting date for you. Please complete & return as soon as possible. Thanks.

In most cases your evaluation of your game will be far more telling than you may realize. This form is to gather information about your golf game, not to grade you on your grammar or knowledge of terminology. Again, this isn't a test.
